Development of community palliative care services has been regarded as one of the top priorities in the healthcare policy agenda worldwide, as well as locally in Hong Kong. The latest published “Hong Kong Cancer Strategy” emphasises on the urge of collaboration among different specialties along the care continuum from hospital to community setting to facilitate care in place. Recently, use of technology to measure patient-reported outcomes can capture patients’ symptoms timely and allow rapid clinical interventions to improve outcomes. The HKU SUPPORT+ mobile app provides up-to-date information about cancer management and palliative care. The app can also monitor symptoms of patients with advanced cancer using standardised tools and empower patients’ capacity on self-management.
